Setting Up Your First Intake Form

Create a professional intake form in minutes using our form builder.

Accessing the Form Builder

  • From your dashboard, click Form Builder in the sidebar
  • You'll see your existing forms (if any) and can create new ones

Creating a New Form

  • Click Create New Form or edit an existing form
  • Choose your form mode:

- Pre-booking — For new lead capture and qualification

- Post-booking — For client onboarding after they've signed

Customizing Your Form

Basic Settings

  • Form Name — Internal name for your reference
  • Welcome Message — Greeting shown to visitors
  • Submit Button Text — Customize the CTA

Adding Fields

Your form comes with default fields. You can:

  • Toggle fields on/off
  • Mark fields as required or optional
  • Reorder fields by dragging

Common Fields Include:

  • Full Name
  • Email Address
  • Phone Number
  • Company Name
  • Project Description
  • Budget Range
  • Timeline

Saving Your Form

Click Save Form to publish your changes. Your form is now ready to share!


Pro Tip: Keep your form concise. Forms with 5-7 fields have the highest completion rates.
